Financial assistance

Our Allina Health Partners Care Program was established to help patients who don't qualify for Medical Assistance or Minnesota Care (MN) or BadgerCare (WI), and whose annual incomes are at or below 275 percent of the federal poverty level. The application is brief and asks for information on family size, employment, income, banking and insurance.

We may ask uninsured patients to apply for coverage through Medical Assistance, Minnesota Care, BadgerCare, or their state’s health exchange (MNSure.org in Minnesota and healthcare.gov in Wisconsin) in order to qualify for Allina Health Partners Care.

Download an application for Allina Health Partners Care (also available in Spanish, Somali, Russian, Hmong, Vietnamese and Chinese). You can also log in to your Allina Health account and apply for financial assistance through the Billing page in your account.

If you have no income, please also download a Statement of Support and have this filled out and signed by the person who provides your food and shelter. The signature must be notarized.

You may scan and email the completed application to AllinaPartnersCare@allina.com or you can fax it to 612-262-3625 or mail it to Allina Health Partners Care, PO Box 43, MR 10209, Minneapolis, MN 55440-0043.
